VPK Updates
The VPK program has been a success in the state of Florida serving over 150,000 children in over 7,000 centers and schools.
The VPK Provider Handbook gives needed information for being a VPK provider.
Provider Application Process
The Early Learning Coalition of Southwest Florida contracts with Community Coordinated Care for Children, Inc. (4C) to register public and private early learning programs as eligible VPK providers. 4C is responsible for collecting, reviewing, and maintaining all required documentation for both public school and private providers. Mail or deliver the completĀed application to 4C, with all required documents. Upon review of required documents you will receive a signed provider agreement authorizing your participation in the VPK program.
Check list for required documentation:
1. One Form AWI-VPK 10 (2/14/2007) per site2. One Form AWI-VPK 11 (5/24/2007) per classroom, a calendar showing VPK days and Non-VPK days
3. Form AWI-VPK 20 Provider Agreement
4. Form AWI-VPK 26 Provider Agreement Addenda

5. Copy of Child Care license or an approved Accreditation or Gold Seal Accreditation
6. Director's Certificate with VPK Endorsement
7. For each school year lead teacher
