2012-2013 VPK Program Forms and Instructions
FALL Provider forms to be submitted to 4C no later than May 31, 2012. If you wish to receive advance payment for the Fall, packets are due no later than May 7, 2012.
The forms below must be completed electronically. Please follow the VPK check list to assure you submit a complete packet. Incomplete paperwork will be returned and there may be a delay in your VPK program start date. If you need assistance completing your paperwork, please make an appointment with the VPK department at 4C by calling 935-6100 (Lee, Hendry, Glades Counties) or 213-1137 (Collier County).

4 Year Old Standards for VPK Teachers and Directors
4 Year Old Standards Training is listed on the DCF website and registration must occur at http://www.dcf.state.fl.us/programs/childcare/about.shtml, course registration.
To download a copy of The Florida Early Learning and Developmental Standards for Four-Year-Olds (2011), go to http://www.fldoe.org/earlylearning/pdf/feldsfyo.pdf and print them. The Standards are NOT distributed at the trainings.
