SR Provider Forms Check List
One completed Rate Form per site (be sure to sign/date)
One completed Holiday Form
One completed SR Contract (English or Spanish) - be sure to initial every page and include attachments if applicable.
One completed SR Application - Fill in electronically. Be sure all blanks are filled in.
One copy of current Liability Insurance with the ELC listed as an additional insured (see item #47 and #48 on SR contract)
Signed Acknowledgement of Understanding Form

All School Readiness Provider Packets should be mailed or dropped off to the ELC.
